Applications should be accompanied with 3(three) most current colored passport size photographs, letter of appointment/promotion to the present substantive grade (where applicable), photocopies of CERTIFIED Academic Transcripts and Certificates, Identity Cards (National), Registration Certificates, Valid Practicing Licenses for Health Workers and, any other relevant documents.
