POSITION SUMMARY

The Case Manager is responsible for overseeing and implementing care for Amani Sasa clients. The manager ensures the programs run smoothly and efficiently. The case manager provides hands-on support to refugee clients experiencing crisis situations, offering holistic care within the Amani Sasa community.

ESSENTIAL DUTIES & RESPONSIBILITIES

Program Oversight and Direct Service

  • Emergency Assistance: Approve and oversee emergency requests within budget and policy.
  • Children’s Welfare: Monitor well-being of scholarship children in coordination with Admin and Logistics Manager.
  • Participant Selection: Oversee intake of clients into the program.
  • Prenatal/Postnatal Support: Mentor young moms through prenatal checkups, delivery, parenting education, and transition support.
  • Mentorship: Conduct ongoing follow-up with participants before and after graduation.

Case Management for Refugee Clients

  • Conduct intake assessments and follow-ups, including home visits.
  • Coordinate services such as legal aid, counseling, and healthcare.
  • Facilitate emergency logistics (e.g., food, housing, transport).

Documentation, Monitoring, and Evaluation

  • Maintain accurate and confidential records.
  • Track changes in client progress through assessments.
  • Submit timely monthly, quarterly, and yearly reports.

Professional Development and Other Duties

  • Attend staff development and continuing education sessions.
  • Represent Amani Sasa in program-related events and community meetings.
  • Perform other duties as assigned by the COO/CEO.

Requirements

  • Bachelor’s degree in Social Work or related field
  • Minimum 3 years of experience in social work, including leadership roles
  • Strong leadership, management and communication skills
  • Experience in program development, implementation and evaluation
